Compensation: Commensurate with experience, starting at $58,000 annually

How you will make an impact

• Conducting assessments of potential residents
• Developing referral networks within the community for available services
• Ensuring a positive resident experience through Welcome Home program
• Assisting residents to develop activities within the community, while encouraging self-sufficiency among residents
• Collaborating with all pertinent departments to coordinate room usage for activities/events
• Maintaining and distributing electronic and paper-based calendars
• Maintaining community event information in the MyEvents system
• Recruiting, orienting and assessing internal and external volunteers to participate in the social programming of the community
• Partnering with the Continuing Care Program Team to seek connections with appropriate volunteer resources, including intergenerational activities and connection to independent living activities.

What you will need 

• Minimum of 3 years of social work experience in a healthcare setting, preferably in senior setting.
• Knowledge of the geriatric population and dementia is preferred.
• Flexibility to attend community events, which may include evenings and weekends
